1. Has any of the persons under this insurance been diagnosed with any Medical Conditions in the past 12 months?
2. Are any of the persons under this insurance been under Consultation or Investigation (apart from check up's with a GP)?
3. Has anyone been hospitalised in the past 12 months?
4. Is anyone under this insurance awaiting any hospitalisation, operations or test results?
5. Has anyone been diagnosed with a terminal prognosis?
6. Has any medication been increased within the past 12 months?
